Case Manager/ Skilled Nursing
We are looking for a Case Manager with experience & passion to enhance the quality of resident management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle and will work as part of a team.
This is an exciting opportunity for an experienced Case Manager to travel between two of our Virginia communities!
Case Management Job Description
Case Manager Responsibilities:
Education:
·Associate degree in the health & human service field required
·Current, active, and unrestricted licensure or certification in a health or human services discipline preferred.
Experience:
·Minimum two (2) years’ experience in medial case management. Comprehensive knowledge of workers’ compensation, insurance, and managed care, required. Ability to negotiate coverage and provide complete and timely case mgmt reports, required. Prior experience with an insurance company, private case mgmt. Company, or HMO, preferred. Familiarity with long- term care and/or sub-acute care, useful. Strong oral and written communication skills, required.
Physical Demands:
·Work is primarily sedentary in nature, with exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull or otherwise move objects, including the human body. Involves sitting, standing, and walking most of the time; also requires driving.
Travel:
·Travel is required between two buildings in Fairfax and Arlington, VA
Principal Contacts:
·Work closely with Administrators, Department Heads, Families, Residents, Physicians, Discharge Planners/Social Workers, Insurance Case Managers, and self-insured employers.
General Requirements:

1.Works in a safety-conscious manner, which ensures that safe work practices are used in order not to pose a risk to self or others in the workplace. Reports any safety violations or hazards to supervisor

4.Interacts in a tactful, diplomatic, and humanistic manner with residents, families, visitors, and co-workers to provide a safe, efficient, and therapeutically effective caring environment which ensures the self-respect, personal dignity, rights and physical safety of each resident and center guest
5.Maintains a dependable attendance record and adheres to standards of cleanliness, grooming, hygiene, and dress code.
